Obtaining Medical Records

HIPAA-square-logoDue to HIPAA (Health Insurance Portability and Accountability Act) regulations, a prison is unable to release medical information or records, known as PHI (Private Health Information) to supporters unless the prisoner has filled out a medical release form and listed one of the supporters as who to release information to.

Note: This form is for Federal prisons only.  State prisons either have their own forms or one may be created using the Federal form as a template.
